When Reece Lapina-Amarmelle was admitted to the Bodiam Ward in Eastbourne DGH, a care plan recommended a long period of sectioning due to his history of self-harm.
However, a discharge meeting was held just nine days later.
Soon after, he took his own life by jumping from Beachy Head.
The story is particularly tragic, because Reece’s 18-year-old brother Kyle had taken his own life eight months earlier by jumping from Beachy Head.
Now NHS trust bosses have said sorry to the family after a three-year fight for justice.
